What causes ADHD?
What research says about genes, brain development, pregnancy and environment in ADHD — and which popular explanations it rules out.

The honest answer is that nobody can point to a single cause of ADHD, and researchers no longer expect to find one. What the evidence describes instead is a condition that emerges from many contributing influences — mostly genetic, some biological, none of them individually decisive.
This is normal for neurodevelopmental conditions, but it frustrates people who want a clean explanation. It also creates space for confident-sounding claims that have no evidence behind them, which is worth keeping in mind when reading about causes anywhere.
Genetics: the largest factor
Twin and family studies consistently find ADHD to be highly heritable, with estimates typically around 70–80%. That makes it one of the more heritable conditions studied in psychiatry — comparable to height in terms of how much variation is explained by inherited factors.
What is inherited is not "an ADHD gene". Hundreds of common genetic variants each contribute a very small amount, alongside rarer variants with larger effects. Heritable does not mean predetermined
High heritability describes what explains differences across a population. It says nothing about whether an individual's outcome is fixed, and it does not mean environment is irrelevant. Support, structure, treatment and circumstances make a substantial difference to how someone's life goes — they simply are not what caused the condition.
Brain development
Imaging studies find average differences between groups with and without ADHD: in the development of some frontal and striatal regions involved in attention and impulse control, in the maturation timeline of certain areas, and in networks handling motivation and reward.
Two cautions matter here. These are group averages with heavy overlap, not a signature you could identify in one person's scan — which is why there is no brain scan that diagnoses ADHD, whatever some private clinics imply. And a difference visible in adulthood cannot easily be separated into cause and consequence.
Factors before and around birth
A number of prenatal and perinatal factors are associated with modestly increased likelihood of ADHD:
- Being born very prematurely or at a very low birth weight — one of the more consistent findings.
- Heavy alcohol exposure during pregnancy.
- Smoking during pregnancy, though studies that compare siblings suggest much of this association reflects shared genetics rather than direct effect.
- Significant lead exposure in early childhood.
- Serious brain injury affecting the frontal regions, which can produce similar difficulties.
Two things are important about that list. These are associations across populations, not explanations for any individual — most people exposed to these factors do not develop ADHD, and most people with ADHD had none of them. And because ADHD is so heritable, an association often reflects shared genetics between parent and child rather than the exposure itself.
What does not cause ADHD
Some explanations have been examined thoroughly and are not supported:
- Parenting. ADHD is not caused by too little discipline, too much indulgence, working parents or divorce. Family stress can make symptoms harder to manage, and having a child with ADHD is itself stressful — a relationship frequently mistaken for a cause running the other way.
- Sugar. Controlled trials in which neither children nor observers knew who had received sugar found no effect on behaviour. What they did find is that parents who believed their child had eaten sugar rated their behaviour as worse.
- Screens and phones. Heavy screen use and ADHD are associated, but the evidence does not establish that screens cause it, and ADHD was described long before either existed. Screens are a plausible consequence of a brain drawn to immediate stimulation.
- Vaccines. No credible evidence supports any link.
- Diet in general. Food additives may have a small effect on hyperactivity in some children, but ordinary diet is not a cause of ADHD.
Why environment still matters
None of the above means circumstances are irrelevant. They shape how much of a problem ADHD becomes:
- A structured job with clear deadlines and a demanding open-plan role can produce completely different outcomes for the same person.
- Sleep, exercise and stress meaningfully affect symptom severity, which is why they feature in non-medication management.
- Early recognition and support change educational and occupational trajectories.
- Whether difficulty gets read as "needs help" or "not trying" affects how someone comes to see themselves.
Common questions
Did I cause my child's ADHD?
No. This is one of the most common worries parents bring to an assessment, and the evidence is clear that ADHD is not caused by parenting. What parents do influence is how well a child copes and how they feel about themselves — which is a considerable amount, but it is not the origin of the condition.
Can trauma cause ADHD?
Trauma can produce difficulties with concentration, restlessness, sleep and emotional regulation that closely resemble ADHD, and the two also co-occur more often than chance. Current evidence does not show that trauma causes ADHD itself. Distinguishing them is a genuinely difficult clinical judgement and a reason assessments ask about history in detail.
Is ADHD caused by a dopamine deficiency?
"Low dopamine" is a popular shorthand, and dopamine signalling does appear to be involved in reward and motivation differences seen in ADHD. But the picture is far more complicated than a simple deficiency, no test measures anyone's dopamine level clinically, and the phrase is often used to sell things. Treat it as a rough metaphor rather than a finding.
